Pelvic tuberculosis is an extrapulmonary form of tuberculosis with increasing incidence in the western world. Clinical and laboratory findings of this disease are often unspecific and mimic a variety of other disorders, including gynecologic malignant tumors. The authors report a case of a 53-year-old woman with pelvic tuberculosis and associated abdominal tuberculous peritonitis. Laboratory investigation included laparoscopy, CA-125 levels and tuberculin test, among others. Discussion on the clinicopathological aspects and diagnostic methods used to elucidate this case is presented.

Paracoccidioidomycosis is an important systemic endemic mycosis in Latin America. This infection is usually acquired via inhalation of mycelial particles. Most infected subjects develop an asymptomatic infection, which is associated with various host-related factors such as sex, age, genetic, as well as characteristics of the infecting agent, mainly its virulence. It is a systemic pathology. A case of mastitis due to paracoccidioidomycosis is presented with the objective to demonstrate that elderly patients with a breast abscess should be submitted to biopsy.

Liposarcoma is a rare neoplasia that originates from primitive mesenchymal cells and is the most common histological type of sarcoma. It mainly affects the limbs but can reach other regions. Genital localization is, however, rare. The authors report a case of vulvar liposarcoma in a 27-year-old patient, who presented a progressively growing mass that limited her movements. Physical examination showed a large ulcerated vulvar tumor. The patient underwent local resection and histopathological examination diagnosed a well-differentiated liposarcoma with uncompromised margins. At the present time, 24 months after surgery, the patient is well and without recurrence of the disease.

A case report of a young virgin patient with a second-degree uterine prolapse is presented. During the corrective surgery (Gillian surgery) samples of the ligaments and the pelvic fascia were obtained to evaluate the elastic fiber system. There were structural changes of the elastic fibers similar to ageing process, which promotes connective tissue weakness inducing a pelvic support defect.

We report a malignant case of breast neoplasm, with the diagnosis of leiomyosarcoma. This rare neoplasm has a less aggressive biological behavior than the other types of breast sarcomas. That is the reason why we emphasize the correct diagnosis, and the necessity of graduating the neoplasm, for the patient's best follow-up. In our case, after 2 years of follow-up the patient continued with no relapse of the disease.

Severe hypertriglyceridemia during pregnancy rarely occurs but it frequently produces complications, such as acute pancreatitis, a serious health risk both for the mother and the fetus. The treatment of a patient who had had acute pancreatitis due to hypertriglyceridemia (triglyceridemia = 5100 mg/dl) two months before fecundation is presented in this paper. During gestation, bezafibrate was substituted for 3.0 g omega-3 fatty acids (14% eicosapentaenoic and 11.13% docosahexaenoic acids). With this therapy, the triglyceride levels were maintained below 800 mg/dl, which is considered to be the safe limit to avoid acute pancreatitis. No complication occurred during pregnancy, the patient delivered vaginally (40 weeks), and the newborn (3075 g) did not present any morphological alterations. We conclude that an adequate diet and the use of omega-3 fatty acids were effective in preventing acute pancreatitis in this pregnant woman with serious hypertriglyceridemia. This therapeutic approach may be used alternatively to other treatments currently utilized for familial hypertriglyceridemia in pregnancy.

A vulvar fibroma, of the molluscum pendulum type, was present in a 20-year-old patient. The tumor began to develop slowly after her menarche, when she was 14 years of age. The physical examination revealed a mass with considerable volume, painless, located at the upper third of the greator left lip, elastic consistency, greater diameter at its distal portion measuring 12 cm by 23 cm in length. The treatment was exeresis from the base of the pedicle, under local anesthesia. The tumor weighed 950 g. A literature review is included.
