    Purpose: high parity and/or young age at first sexual intercourse have been reported as risk factors for cervical cancer development. Actually, little research has been devoted to these risk factors. The aim of the present study was to analyze the parity and age at first sexual intercourse in women with invasive cervical cancer. Methods: we have retrospectively studied the age at first sexual intercourse and parity of 362 women with invasive cervical cancer diagnosed at the Outpatient Clinic of Oncological Gynecology of the "Faculdade de Medicina do Triângulo Mineiro" from 1978 to 1995. The cases were analyzed according to the date of diagnosis and divided into three groups: from 1978 to 1983 with 65 cases, from 1984 to 1989 with 127 cases and from 1990 to 1995 with 170 cases. Results: the results showed that high parity (4 or more births) diminished from the first to the third period (82, 67.3 and 63.8%, respectively) (p<0.02, chi² test). The first sexual intercourse before 18 years occurred respectively in 59.2, 54.5 and 55.5% of the patients (p, not significant). Conclusions: it was concluded that high parity seems to be related to invasive cervical cancer, although in recent years the parity decreased. The first sexual intercourse occurred before the age of 18 years in the patients of the three periods.

    PURPOSE: to estimate the validity of visual inspection of cervical intraepithelial neoplasia (CIN) and HPV-induced lesion screening, after acetic acid application (VIA), and to compare its performance with that of colpocytology and colposcopy. METHODS: a diagnostic test validation study involving 893 women aged 18 to 65 years, simultaneously screened with colpocytology, VIA and colposcopy was carried out at a public health unit in Recife, PE. VIA was performed by applying 5% acetic acid onto the cervix and observing it with the help of a clinical spotlight. The finding of any aceto-white lesion on the cervix was considered positive. The gold standard was the histopathology of cervical biopsy, carried out whenever any of the three test results was abnormal. Validity indicators were estimated for each test, within 95% confidence intervals. The analysis of agreement between test results was done by the kappa coefficient. RESULTS: of 303 women submitted to biopsy, the histopathological study was abnormal in 24. Among this total, VIA was positive in 22, yielding an estimated 91.7% sensibility, 68.9% specificity, and 7.5% positive predictive value and 99.7% negative predictive value. Comparing 95% confidence intervals, VIA was more sensitive than colpocytology, despite a lower specificity and positive predictive value. There was poor agreement between VIA and colpocytology (k=0.02) and excellent agreement with colposcopy (k=0.93). CONCLUSION: VIA was much more sensitive than colpocytology in the screening of CIN and HPV-induced lesions and presented a performance similar to colposcopy. Its low specificity determined a high number of false-positive results.

    OBJECTIVE: to evaluate staging, surgical procedures and outcome of ovarian granulosa cell tumors (GCT), with emphasis in the possibility of conservative surgery (unilateral ooforectomy). METHODS: this is a retrospective cohort study in which 24 patients treated with GCT during the period of January 1994 to January 2004 were included. Variables of interest such as patient's age, symptoms, tumor size on physical examination, staging, treatment modalities (types of surgery and of adjuvant chemotherapy and/or radiotherapy) and prognosis have been analyzed. The chi-square test was used to check the association between variables, and the level of significance was set at p < 0.05, always describing the two-tailed Fisher exact test. RESULTS: the patients' age varied from 30 to 82 (mean 51.7) years old. The most frequently referred symptom was pelvic pain (n = 10; 41.7%). Fourteen patients (58.3%) presented clinical stage (CS) Ic, 5 (20.8%) CS IIIc and 5 (20.8%) CS Ia. More advanced clinical stages were significantly associated with palpable pelvic masses (p < 0.04). Endometrial hyperplasias were found in 3 (25%) of the 12 hysterectomy specimens. The follow-up period varied from 2 to 114 (mean 42.5) months. Among the 16 (66.7%) surviving and disease-free patients, 4 had CS Ia (25%), 11 CS Ib (68.8%) and 1 CS Ic (6.3%), whereas all patients who died had CS IIIc (p = 0.0008). All 6 patients (25%) that underwent conservative surgery were alive and without recurrence. CONCLUSION: although GCT usually present low aggressiveness, the clinical staging continues to be the main prognostic factor and hence it determines the option for conservative surgery and the use of adjuvant therapy.

    PURPOSE: to evaluate the local, regional and distant recurrence rate of a new surgical technique for the conservative treatment of the breast cancer in early stages. The technique is based on breast segmental resection with axillary dissection and skin sparing by a single periareolar incision. METHODS: one hundred and nineteen patients with breast cancer stages I and II constituted the present study. The study group comprised fifty-seven patients who were submitted to surgery by the proposed technique, while 62 patients submitted to the classic quadrantectomy constituted the control group. Postoperative radiotherapy and boost were perfomed in both groups. The average follow-up was 50.1 months for the study group and 51.2 months for the control group. The rates of recurrence, global survival and disease-free survival were analyzed and compared between the two groups. RESULTS: the rate of local recurrence in the period was 3.5% for the study group and 4.8% for the control group. There was no statistically significant difference between the groups as to disease-free survival and global survival rates. CONCLUSION: we demonstrated that the technique of segmental resection with axillary dissection by a single periareolar incision, preserving skin, did not show difference in relation to the classic quadrantectomy, regarding rates of recurrence, global survival and disease-free survival.

    Purpose: to evaluate the prognostic value of estrogen receptor and p53 and c-erbB-2 proteins in lymph node-negative breast cancer. Methods: an immunohistochemical study was made in paraffin-embedded tissues from the file of the Instituto de Pesquisas Cito-Oncológicas of the Fundação Faculdade Federal de Ciências Médicas de Porto Alegre of fifty cases of postmenopausal women, who were treated at the Irmandade da Santa Casa de Misericórdia de Porto Alegre and at the Santa Rita Hospital from 1990 to 1994. For statistical analysis c² with Yates correction, as well as exact Fisher tests were used and Kaplan Meier curves compared with log-rank test. The mean follow-up of the patients was 3.6 years (3.1-4.5). Of the 50 cases, 14 showed recurrence during the period of follow-up. Results: the mean age was 61 years (46-78). Modified radical mastectomy was performed in 35 patients (70%) and 15 (30%) were submitted to lumpectomy/axillary dissection and postoperative radiation therapy. Fifty percent of the patients who showed recurrence did it in the first three years after the diagnosis. The mean size of the tumor was 2.8 cm (1.98-3.13) and the most frequent histological type was invasive ductal carcinoma of no special type (92%), according to the Bloom and Richardson graduation, 3 being stage I (6.6%), 35 stage II (76%) and 8 stage III (17.4%). In the tumors with recurrence, there was no grade I, 9 stage II (25.7%) and 3 stage III (37.5%). In relation to the prognosis, the disease-free interval was less when there was association of a poorly differentiated tumor with negative estrogen receptor (p = 0.006), positive p53 (p = 0.006) and positive c-erbB-2 (p = 0.001). Conclusion: postmenopausal women with lymph node-negative breast cancer showed worse prognosis in relation to disease-free interval when they presented poorly differentiated tumor associated with negative estrogen receptor, positive p53 and positive c-erbB-2.
