To assess the association between sociodemographic and perinatal factors and hospital practices to encourage exclusive breastfeeding in near miss neonates in maternity hospitals.
This is a prospective cohort of live births from the survey “To be born in Brazil” conducted between 2011 and 2012. The weighted number of newborns who met the neonatal near miss criteria was 832. Exclusive breastfeeding at hospital discharge and 45 days after delivery were dependent variables of the study. The sociodemographic and perinatal factors of the puerperal women and hospital practices to encourage breastfeeding were independent variables. The data were analyzed with Poisson regression and set with p value<0.05. Is exclusive breastfeeding in neonatal near misses associated with factors related to sociodemographic conditions, maternal characteristics and the organization of health services?
Data from 498 women and their children were analyzed. Mothers with incomplete primary education were more likely (36%) to have exclusive breastfeeding (RR: 1.36; 95% CI: 1.06-1.74) at discharge. Women who did not offer the breast to the newborn in the joint accommodation (65%) were less likely to be breastfeeding exclusively (RR: 0.65; 95% CI: 0.56-0.75) at discharge. Variables that increased the probability of exclusive breastfeeding after 45 days of delivery were primiparity (RR: 1.36; 95% CI: 1.08-1.69) and having the newborn in the delivery room (RR: 1.90; 95% CI: 1.12-3.24).
Exclusive breastfeeding in neonatal near misses was associated with maternal characteristics and important hospital practices, such as being breastfed in the joint accommodation and the newborn being in the mother’s lap in the delivery room.

To evaluate the effect of supplementation with omega-3 sources on the fatty acid composition of human milk.
The review consisted of the search for articles published in PubMed, Biblioteca Virtual de Saúde (Virtual Health Library[VHL]) andWeb of Science databases using the following keywords: fatty acids, omega-3, human milk and supplementation; for this purpose, we have used the program of research to integrate the services for the maintenance of autonomy (PRISMA) checklist. The following selection criteria were used: articles in English, Portuguese, Spanish or Italian, published between 2000 and 2015, and about studies performed in humans. We found 710 articles that met the established criteria; however, only 22 of them were selected to be part of this study.
All studies found a positive relationship between the consumption of omega- 3 sources and their concentration in humanmilk. The differences in the findings are due to the distinct methods used, such as the specific time of the omega-3 supplementation, the type of omega-3 source offered, as well as the sample size.
Although the studies were different in several methodological aspects, it was possible to observe the importance of omega-3 supplementation during gestation and/or the puerperium.

PURPOSE: To evaluate sociodemographic, behavioral and reproductive factors and morbidities associated with inadequate weight gain during pregnancy. METHODS: Cohort study conducted from December 2007 to August 2008 with women in the first trimester of pregnancy looking for prenatal care in the Public Health System who lived in the cities of Petrópolis or Queimados, Rio de Janeiro state (Brazil). Women with multiple pregnancy, who had a miscarriage in the index pregnancy or who lacked information for the assessment of pregravid nutritional status or weight gain were excluded. Pregravid nutritional status and weight gain during pregnancy were determined according to the criterion established by the Institute of Medicine (IOM). Statistical analysis was performed using a multinomial logistic regression model. RESULTS: A total of 1,287 women were included in the study; 26.6% of them were overweight or obese while 11% were underweight. Inadequate weight gain during pregnancy was observed in 71.4% of pregnant women; 35.6% of them did not gain enough weight while 35.8% gained more weight than recommended by the IOM. In the multivariate analysis, women with hypertension (OR=2.1; 95%CI 1.4-3.1), pregravid overweight (OR=2.5; 95%CI 1.4-4.5) or obesity (OR=2.7; 95%CI 1.8-3.9) and who had a higher educational level were more likely to gain more weight than recommended, while pregravid underweight (OR=0.6; 95%CI 0.4-0.9) represented a protection against excessive gain. CONCLUSION: Pregravid nutritional diagnosis and weight gain monitoring should be actions effectively instituted in the routine of health professionals.

PURPOSE: to determine caffeine consumption in pregnant women and to evaluate its association with demographic, socioeconomic, reproductive, lifestyle and maternal nutritional status. METHODS: it is a cross-sectional study performed between 2005 and 2007. The present analysis refers to the period among the 8th and 13th gestational week and included 255 pregnant women from 18 to 40 years, clients of a municipal health center in Rio de Janeiro. The outcome variable was caffeine consumption, quantified by a semi-quantitative food frequency questionnaire, which count with a list containing 81 items and eight options of consumption frequencies; besides it being previously validated in a sample of employees of the State University of Rio de Janeiro. The caffeine intake was quantified starting from the consumption of: powdered chocolate, chocolate bar or chocolate, soft drink, coffee and mate tea. The statistical analysis was performed by means of fitting a multivariate linear regression. RESULTS: the median and the mean caffeine consumption were, respectively, 97.5 and 121.1 mg (standard deviation, sd = 128.4). The high caffeine consumption (> 300 mg/day) was observed in 8.3% of pregnant women. It was observed in the multivariate model that women with earlier menarche (β = -0.15), with more household partners (b = 0.17) and who didn’t make use of medicines (β = -0.24) presented larger tendency to high caffeine consumption association that was statistically significant (p <0.05). CONCLUSIONS: the caffeine consumption for most of the pregnant women was inferior to the limit of 300 mg/day as commited in other studies. Tendency was observed toward higher consumption of caffeine in pregnant women with earlier menarche, with more household partners and who didn’t make use of medicines.
