PURPOSE: to assess the natural pregnancy rates in rabbits submitted to bilateral oophorectomy and orthotopic autologous (both intact and sliced) ovarian transplantation without vascular pedicle and to verify the morphofunctional aspects of reimplanted ovaries. METHOD: thirty-two female New Zealand White rabbits were studied. The ovaries were removed and orthotopically replaced without vascular anastomosis. In the control group (GC) (n=8), only laparotomy was carried out. In the RI group (n=8) intact ovaries were reimplanted in both sides. In the RF group (n=8) the ovaries were sliced and orthotopically reimplanted. In the RIF group (n=8), in one side the intact ovary and in the other side the sliced ovary was reimplanted.Three months later, the animals were paired with sexually mature males for copulation. Estradiol, progesterone, follicle stimulating hormone and luteinizing hormone levels were assessed nine months after surgery. Histologic study of the transplanted ovaries, tubes and uteri was carried out, and the number of pregnancies and animals per litter in each group was taken into account as well. The chi² test compared the pregnancy and the litters between the groups. Student's t test compared the hormone determinations. Significance was set at p<0.05. RESULTS: pregnancies occurred in all rabbits of the GC, between the second and third months after copulation, with 6 to 10 animals per litter for animal. In the other groups, the pregnancies occurred between the fifth and eighth postoperative month. Pregnancies occurred in 37.5% of rabbits in group RI, and in 50% of the RF and RIF groups. There was no difference between the number of animals per litter in the reimplanted groups, with 1 to 4 animals per litter for each animal. Hormone levels and histology confirmed the vitality of all ovaries. CONCLUSIONS: intact or sliced orthotopic autologous ovarian transplantation without vascular pedicle is viable in rabbits, and preserves the hormonal and fertile functions.

The purely mucinous tumors of the ovary are considered, by some authors, as a monodermic variant of mature teratoma. Few case reports mention the coexistence of these two tumors with distinct cellular origin. Our objective is to describe the case of a 38-year-old patient with a mucinous cystadenocarcinoma with low malignant potential arising in a mature cystic teratoma and to divulge the importance of the knowledge about this association to the gynecological surgeon.

PURPOSE: to estimate the sensitivity, specificity and accuracy of patient age, ultrasound result and CA-125 marker variables for the differential diagnosis between malignant and benign ovarian tumors. In addition, to establish a risk of malignancy index (RMI) incorporating these three variables and to estimate its sensitivity, specificity and accuracy for the differential diagnosis. METHODS: one hundred patients with ovarian tumors with surgical indication were included. The age, ultrasonographic findings and CA-125 level variables were evaluated separately and later on together as the RMI. The study was performed based on the evaluation of the sensitivity, specificity and diagnostic accuracy and the use of the measurements: likelihood ratio, odds ratio, and the Student's t test, chi², and logistic regression with univariate and multivariate analysis. RESULTS: for the age variable, sensitivity, specificity and diagnostic accuracy were 58.8, 68.2 and 65.0%, respectively. For ultrasound, 88.2, 77.3 and 81.0%. For CA-125 dosage, the values were 64.7, 74.2 and 71.0%. When the three variables were put together, as the RMI, a sensitivity of 76.5%, a specificity of 87.9% and a diagnostic accuracy of 84.0% were observed. CONCLUSIONS: RMI, made up of the association of patient age, ultrasound results and CA-125 dosage variables is a valuable indicator to distinguish between malignant and benign ovarian tumor, especially in regard to its specificity.

Purpose: to compare ovarian adhesion according to different types of ovarian cortex closure. Methods: fifteen rabbits were randomly assigned to 3 groups according to the type of ovarian cortex closure, consisting of bipolar diathermy, 5-0 polyglactin 910 and 5-0 plain catgut. After 2 weeks the animals were sacrificed and the adhesion scores were evaluated in the right ovary using the Diamond classification. The left ovary, not incised, was used as control. Statistical analyses were done using analysis of variance (ANOVA), Scheffé test and Student's t test. Results:the mean adhesion scores for the bipolar, polyglactin and plain catgut groups were 0.7, 1.5 and 2.0, respectively. The analysis of variance detected a significant difference (p=0.02) between the three groups. Using Scheffé test for the two by two comparison of the groups, a significant difference was found between the bipolar and the plain catgut groups. Comparing the group in which sutures were used (independent of the material employed) with the bipolar group, a statistically significant difference was observed (1.8 and 0.7, respectively, p=0.01). Conclusions: the results obtained in this study are in accordance with the literature data, showing that second intention closure is more advantageous, concerning adhesion formation, than closure using sutures.

Luteomas of pregnancy are ovarian pseudotumors diagnosed by ultrasound, during cesarean section or at postdelivery tubal ligation. Twenty-five per cent of the cases appear around the second half of pregnancy. Usually there are signs of maternal virilization and 50% are detected because female newborns show clitorimegaly and/or labial fusion. The concentrations of androgenic steroids in the maternal blood during pregnancy and in the cord blood at child-birth show significantly increased rates. The ultrasound shows solid or cystic-solid structures and few weeks after the delivery they decrease and the ovary size returns to normal. The authors report a case of a patient who exhibited virilization signs in two consecutive pregnancies as well as in the two female fetuses. At adnexal sonographic examination a solid tumoral image was found in both pregnancies. Serum androgen levels were increased.

Brenner's tumor is small, unilateral and benign. Its diagnosis can only be confirmed by the anatomomicopathological examination, that on microscopy presents nests or columns of transition epithelial cells in a fibromatous matrix. The epithelial cells present a characteristic longitudinal "coffee bean" furrow and can undergo carcinomatous transformation. We present a case of a menopausal patient with vaginal bleeding, and a palpable abdominal mass. She was submitted to surgical treatment (total hysterectomy, bilateral oophorectomy and omentectomy) with the histopathological diagnosis of Brenner's bilateral tumor, being malignant on one side and benign on the other. After surgical treatment she is now alive for three years and free of disease.

Purpose: to evaluate the effect of total abdominal hysterectomy on ovarian blood supply in women in reproductive age, using transvaginal color Doppler. Methods: a prospective study was carried out on 61 40-year-old or younger women. They were divided into two groups: G1, 31 patients submitted to hysterectomy, and G2, 30 normal women. Criteria of inclusion: normal ovarian function at baseline, normal body weight, without expasure to tobacco, no history of laparotomy or ovarian pathology. The pulsatility index (PI) of ovarian arteries determined by Doppler and the transvaginal ultrasonographic measurement of the ovarian volume were performed at three moments: baseline, 6 and 12 months. Results: at baseline the groups were homogeneous. In the hysterectomized group, with 6 and 12 months, increase of the volume of the ovaries and decrease of PI (p<0,05) were observed. At the end of the study, 25.5% (8/31) of the hysterectomized patients presented benign ovarian cysts. In the control group no alteration of the parameters was observed. Conclusion: The decrease of PI by Doppler of ovarian arteries suggests increased ovarian blood supply after total abdominal hysterectomy in women in reproductive age.

Purpose: in order to maintain the gonadal function after oophorectomy, morphofunctional aspects of ovarian autotransplantation to the greater omentum and the best kind of implantation, intact or sliced, were investigated. Methods: forty cycling female Wistar rats were randomly divided into four groups: Group I (n = 5), control - laparotomy; Group II (n = 5), bilateral oophorectomy; Group III (n = 10), intact ovarian autotransplantation to the greater omentum; and Group IV (n = 10), sliced ovarian autotransplantation, both to the greater omentum. The estrous cycle was investigated in the third and sixth postoperative months and histological studies of the ovarian implants were carried out considering: degeneration, fibrosis, inflammatory reaction, angiogenesis, follicular cysts, follicular development and corpora lutea. Results: the animals of Group I preserved the cycling sequence. The rats of Group II remained in diestrus. In Group III, 11 rats remained in diestrus, three presented incomplete cycles and one showed normal cycle. In Group IV, three animals remained in diestrus, eight showed incomplete cycles and four showed normal cycles. The histology of the ovaries of Group III was normal in ten female rats; however, the ovaries of the other five animals presented degeneration. In Group IV, 14 female rats had ovaries with preserved morphological aspect, and signs of degeneration occurred in one. Conclusions: the ovarian autotransplantation to the greater omentum is viable and the sliced form presented better morphofunctional aspects than the intact implants.
