To identify obstetric and perinatal factors associated with perinatal morbidity and mortality in pregnancies that progressed with ruptured membranes.
A cross-sectional hospital-based study with secondary data from records of patients (n=87) that evolved with the premature rupture of membranes between 24 and 42 weeks of gestation, admitted from January to April 2013 to a public hospital in Acre State, North of Brazil. Data were subjected to bivariate analysis for selection of variables to be used in a multiple regression model according to Poisson logistic regression with robust error.
The prevalence of perinatal morbidity-mortality was 51.4%, including a 2.3% death rate (2 cases) and a 9.2% fetal neonatal death rate (8 cases). The variables associated with mortality in the final multiple model were: number of prenatal consultations ≥6, with a prevalence ratio (PR) of 0.5 and a 95% confidence interval (95%CI) of 0.3-0.9, gestational age ≥30 weeks (PR=0.6; 95%CI 0.4-0.8), low birth weight (PR=2.9; 95%CI 1.5-5.4), and mechanical ventilation (PR=3.8; 95%CI 2.0-7.2).
Perinatal morbidity and mortality were high among cases of ruptured membranes. Morbidity and mortality were associated with factors such as fewer prenatal visits, extreme prematurity and low birth weight in this group.

PURPOSE: To analyze the perinatal mortality rate in cases of gastroschisis and possible associated factors. METHODS: A retrospective cohort study was conducted between 1992 and 2012. All cases of gastroschisis born in Hospital de Clínicas de Porto Alegre (HCPA) during that period were included. The diagnosis of gastroschisis was obtained by morphological ultrasound examination or clinical examination at birth in prenatally unknown cases. The variables of birth (birthweight, gestational age and Apgar score, mode of delivery, type of gastroschisis and associated anomalies) and the surgical ones (type of surgical closure, reintervention and sepsis) were compared between surviving cases and deaths. The results of this comparison were analyzed according to the type of variable using parametric and non-parametric tests (Mann-Whitney or Student's t-test, χ² or Fisher's exact test), with the level of significance set at 5% (p=0.05). RESULTS: Sixty-four newborns with gastroschisis were included, 59 of them (92.2%) diagnosed during the prenatal period. Twenty-six patients (40.6%) had only exposed intestines, classified as simple gastroschisis, 22 had exposure of the intestines and stomach (34.4%) and 16 had exposure of the intestine and other organs (25%), for a total of 38 cases of complex gastroschisis. Primary surgical repair was performed in 44 cases (68.8%). The mortality rate was 23.4% (15 deaths). Babies who died had significantly lower birth weight (p=0.001), gestational age (p=0.03) and Apgar score (p=0.03) than survivors. There was no difference in mode of delivery (p=0.8) and, with respect to gut contents, there was no difference between the cases of simple and complex gastroschisis (p=0.06). Mortality was significantly higher in patients with sepsis (p=0.008) and reintervention (p=0.001). CONCLUSION: in the present study, perinatal mortality due to gastroschisis seemed to depend mainly on prematurity, low birth weight, and surgical complications.

To investigate the association of perinatal variables with the birth of very low birth weight (VLBW) preterm newborns.
It was a retrospective study of the medical records of infants born after spontaneous preterm labor with admission to a neonatal intensive care unit. Preterm infants were divided into two groups: very low birth weight (VLBW) group (weight <1,500 g) and low birth weight (LBW) group (weight ≥1,500 g and <2,500 g). Prenatal variables such as maternal complications during pregnancy and childbirth/postpartum, and fetal/neonatal complications were investigated. Statistical analysis was performed using the Fisher exact test or χ2 test, with calculation of relative risk (RR), and the Student t test for comparison of group means, with the level of significance set at p≤0.05.
Hemorrhagic comorbidities (p=0.006; RR=1.2) and hypertension (p=0.04; RR=1.5), surgical delivery (p=0.001; RR=0.5), gestational age <33 weeks (p< 0.001; RR=16.7) and Apgar score at 1st and 5th minute (p=0.006; RR=1.6; p=0.01; RR=1.9) were associated with the occurrence of VLBW. Infants with VLBW had a significant association with the occurrence of metabolic comorbidities (p=0.01; RR=1.8), neurological (p=0.01; RR=1.7) and infectious diseases (p=0.001; RR=1.9), hospitalization >4 weeks (p=0.02; RR=1.8) and early neonatal death (p=0.0001; RR=2.9).
Factors such as hypertension and bleeding comorbidities during delivery and management of gestational age of less than 33 weeks were associated with the birth of VLBW newborns. This group of infants also showed higher RR for the occurrence of early neonatal death.

PURPOSE: to apply geoprocessing techniques for the spatial birth profile analysis of each municipality. METHODS: ecological and exploratory study, using data from the Health Information System about born alive babies in 2004, and using geoprocessing techniques. The spatial autocorrelations of the variables: cesarean section, mother's schooling, low birth weight, Apgar score at five minutes, prematurity, number of medical appointments and adolescent mothers, besides the map with the index of human development were estimated. For the detection of spatial events aggregates, Moran's I M statistics, through the program Terra View 3.13 (developed by INPE and available to the public) was used. Spatial maps with those variables were built, and Pearson's correlation coefficients, estimated. RESULTS: results have shown that the rate of born alive babies, from mothers with school level over primary school and from cesarean sections, presented a spatial pattern visually identifiable and significant spatial self-correlation. Low birth weight, prematurity, Apgar score, number of pre-natal appointments and adolescent mothers have presented a random spatial pattern, showing that, in this analysis scale, those markers have not discriminated the risk groups, despite their unquestionable predictive value for children's morbidity-mortality at individual level. There has been a positive correlation between cesarean section and schooling, and between cesarean section and human development index; and a negative correlation between adolescent mothers and human development index, with statistical significance (p<0.05). CONCLUSIONS: this methodology has allowed us to identify spatial clusters for the variables cesarean section and mother's schooling, besides deepening our knowledge on birth profile in the municipalities, presenting good potential on how to direct actions for specific areas.

PURPOSE: to analyze the association of pregnancy in adolescence with low birth weight (LBW). METHODS: all the patients who gave birth in a teaching Hospital at Maranhão State from July to December 2006 were included in the study and were split in two groups: adolescents (ten to 19 years old) and adults (20 to 34 years old). The variables studied were: skin color, schooling, marital status, family income, number of pre-natal appointments, gestational age at the onset of pre-natal assistance, place where they spend that period, gestation duration, delivery route and birth weight. Data were processed by the Epi-Info program, 3.4.1 version, and the associations between variables were analyzed by the Odds Ratio (OR), with a confidence interval (CI) of 95%. Models of logistic regression were also used. The significance level adopted was 0.05. RESULTS: 1,978 patients were evaluated. A ratio of 25.4% of deliveries in adolescents was observed; they presented low level of schooling, no mates, low number of pre-natal appointments, late onset of pre-natal assistance, low birth weight and prematurity. In the analysis of LBW as end variable, associated to prematurity (OR=29.0), it was clear the association with low number of pre-natal appointments (OR=2.98; 95%CI=2.23-4.00), pre-natal late onset (OR=1.91; 95%CI=1.3-2.6) and low schooling (OR=1.95; 95%CI=1.4-2.5) related to adolescence (OR=1.50; 95%CI=1.1-1.9). Similar results were obtained when the prematurity variable was excluded. Adolescents showed lower incidence of caesarean section (33.3%) than adults (49.4%), a significant difference, besides lower association with pre-eclampsia and cephalo-pelvic disproportion. CONLUSIONS: pregnancy in adolescence was associated to pre-natal late onset and low number of appointments, besides low schooling, low birth weight and a lower incidence of cephalo pelvic disproportion and pre-eclampsia.

PURPOSE: to analyze birth weight in a cohort of newborns for the year 2000, in Goiânia, by determining the coefficient of mortality and neonatal survival probability, stratified by categories of birth weight, and also, through the identification of factors associated with low birth weight (LBW). METHODS: a retrospective cohort study, made possible by the linkage of data from the ISM (Information System on Mortality) and ISLB (Information System on Live Births) files. Coefficients of neonatal mortality were calculated for the categories of birth weight and a neonatal survival probability chart was constructed with the help of linear regression analysis. Risk factors for LBW were identified by univariate analysis (RR) and logistic regression analysis, and the level of significance was set at 5%. RESULTS: the incidence of LBW was 6.9% and 140 (66.8%) neonatal deaths took place in this group. Thirty percent of these deaths occurred in the 1,500-2,500 g weight bracket. The following risk factors were identified for LBW: preterm pregnancy, presence of congenital malformations, mothers at the extreme ages for reproduction, mothers living in the northwestern region of the city, insufficient prenatal appointments with the doctor, delivery in a public hospital, and female babies. CONCLUSION: Goiânia had an incidence of LBW which is comparable to that of developed countries and coefficients of neonatal mortality by category of weight were below those found for those countries. These results recommend that we pay attention to: prematurity, public hospitals, and the northwestern region of Goiânia.

PURPOSE: to verify if there is any difference in the number of miscarriages and newborns with low weight in patients with scleroderma (SSc) when compared with women without the disease, between the two clinical variants of the disease and when the expression of SSc occurred before or after the gestation. METHODS: twenty-six patients were analyzed regarding the number of pregnancies, miscarriages, sex, and birth weight of the children, studying the clinical variant of the disease and the temporal relationship between diagnosis and gestation. The patients had not used either medications that could interfere in gestation or had any other disease that could do it. For control, twenty-six healthy women, without diseases that might alter the gestation, of the same ages and socioeconomic status were studied. For the statistical analysis study, tables of frequency, tables of contingency, and Fisher, chi2 and Mann-Whitney tests were used. Statistical significance was considered when p<0.05. RESULTS: among the patients with SSc, there were 96 gestations with 13.5% (n=13) of miscarriages. In the control group, there were 94 gestations with 9.6% (n=9) of miscarriages. There were no differences in the number of newborns with low weight between the two groups (cases, n=8 and controls, n=6, with p=0.54), nor in the number of miscarriages (p=0.46). However, the number of newborn babies with low weight was significantly higher among the cases with the diffuse disease (diffuse form, n=4 and limited, n=4, with p=0.04) and among patients that became pregnant after the diagnosis of SSc (37.5% in women known to be sick and 6.7% in women who became pregnant before getting ill, with p=0.03). The number of male newborns was higher in the women with SSc (p=0.002). CONCLUSIONS: the women with SSc showed a higher number of low-weight newborns in the group with diffuse disease and when pregnancy occurred after the clinical diagnosis of the disease.

PURPOSE: to evaluate periodontal conditions and need for treatment supplied by Periodontal Screening and Recording (PSR) in mothers, in order to clarify the relationship between periodontal disease and low birth weight premature newborns. METHODS: PSR was used in a sample of 40 mothers, divided into: test group, consisting of mothers of premature newborns with weight less than 2,500 g (n=20), and the control group, consisting of mothers of term newborns with a weight equal to or over 2,500 g (n=20). The collected data were analyzed by descriptive statistics, and the results of PSR were submitted to statistical analysis in order to verify differences in periodontal condition and need for treatment of the mothers, using the Kolmogorov-Smirnov test, with a significance level of 5%. RESULTS: the presence of periodontal pocket of 3.5 to 5.5 mm was the most common finding among the mothers of newborns with low weight (39.2% of the cases), while the presence of bleeding at probing and absence of a periodontal pocket were the most frequent findings among the mothers of newborns with normal weight (37.5% of the cases). There was a statistically significant difference between periodontal conditions of the mothers of the groups (p=0.0494), but in relation to the need for treatment, there was no significant difference between the studied groups (p>0.05). CONCLUSION: the mothers of preterm newborns with low weight presented worse periodontal conditions, suggesting that periodontal infection may be related to preterm low birth weight newborns.
