PURPOSE: to evaluate the impact of supplementary ferrous sulfate and dietary counseling on hemoglobin levels in pregnant women. METHODS: a total of 197 pregnant women were evaluated during antenatal care at a health center. The treatment group consisted of 105 women who were prescribed 60 mg dietary iron per day, received dietary counseling and had hemoglobin measured by a portable photometer between the 14th and 20th week of pregnancy. The treatment group was reevaluated according to hemoglobin levels and food intake by a semiquantitative food frequency questionnaire after the 34th week of pregnancy. The control group consisted of 92 women in a cross-sectional study, at no less than 34 weeks of pregnancy. Hemoglobin was analyzed by a portable photometer and anemia was defined concentrations of less than 11 g/dL. All pregnant women had their weight and height measured. Hierarchical logistic regression model was developed for the multivariate analysis. RESULTS: prevalence of anemia at the end of the third trimester was 31.6% in the treatment group and 26.1% in the control group (p=0.43). Use of the prescribed supplement was reported by 65% of women in the treatment group, of which 67.7% interrupted the treatment at some point. Principal reasons for interrupting treatment were forgetting (43.2%) and nausea or vomiting (27.2%). Risk of anemia in the third trimester was three times higher in women with less than 8 years of schooling. CONCLUSIONS: use of ferrous sulfate was not shown to be associated with lower prevalence of anemia. The results suggest that structural changes in socioeconomic conditions are needed in order to alter the current situation regarding iron deficiency anemia.

PURPOSE: to evaluate, in pregnant adolescents, the incidence of iron deficiency, using the following blood tests: hemoglobin, ferritin, serum iron, transferrin saturation rate and serum transferrin receptor, and their relationships. METHODS: a total of 56 adolescents were included at the first prenatal evaluation between the 12th and the 20th week of gestation. The normal values for each test were: above 11 mg/dL for hemoglobin, 12 µg/dL for ferritin, 50 mg/L for serum iron, 16% for transferrin saturation rate and below 28.1 nmol/L for serum transferrin receptor. Each result was evaluated using percentages and the McNemar test was used to compare the results. RESULTS: incidence of anemia using the hemoglobin concentration test was 21.4%. All pregnant women presented mild anemia. In the present study, 21.4% of the patients had iron deficiency with a ferritin concentration <12 mug/dL. Serum iron concentration was reduced in 3.6% of the adolescents and transferrin saturation rate in 26.8% of the sample. The value obtained by the transferrin receptor test was unclear, due to the lack of international standardization regarding measure unit. Comparing the hemoglobin concentration test to the other iron deficiency tests, it was found that the latter do not show a better evaluation than the hemoglobin concentration test in patients with hypoferremia. CONCLUSIONS: the hemoglobin concentration test in patients with mild anemia was effective to identify iron deficiency.

PURPOSE: to describe the frequency of clinical and laboratorial findings in pregnant women with malaria. METHODS: a descriptive study was performed including 445 pregnant women with a positive diagnosis of malaria of the Maternity and women's Clinic Bárbara Heliodora (Rio Branco, Acre State), from January 1996 to December 2001. These cases were reviewed and the pregant women with malaria were selected. RESULTS: a total of 33,420 patients were hospitalized in that period. Of these, 445 pregnant women (1.4%) were included. Among these, the frequency of the infection with Plasmodium vivax was 52.8% (n=235), with P. falciparum, 43.8% (n=195), and 3.4% (n=15) with both plasmodia. The most frequent clinical and laboratorial alterations (p<0.05) were observed in the patients with P. falciparum: pale mucosa, jaundice hemoglobin and hematocrit decrease, hypoglycemia and increase in serum aminotransferase, urea, creatinine and in bilirubin levels. Only one patient (1/445), a pregnant woman infected with P. falciparum died. Abortions, premature birth and low birth weight were detected in 1.3, 1.1 and 1.1% of the cases, respectively. CONCLUSIONS: these findings reflect the repercussion of malaria during pregnancy, and justify the existence of qualified medical care in obstetric units for diagnosis and treatment of these cases, especially in the Amazon region.

Purpose: to determine the frequency of enteroparasitoses in a group of pregnant women undergoing low-risk antenatal care and their association with anemia, maternal nutritional status, schooling and the existence of a bathroom in the home. Methods: to a sample of pregnant women who had begun low-risk antenatal care at IMIP's Maternal Health Care Center between May 2000 and July 2001, a cross-sectional design was applied to determine the frequencies of enteroparasitoses (Hoffman method, in a single sample) and anemia (Hb <11.0 g/dL), nutritional status (through BMI standardized for stage of pregnancy) and social indicators (schooling and the existence of a bathroom in the home). Results: in a sample of 316 pregnant women, a rate of 37.4% enteroparasitosis was detected, of which 31.6% was infestation by a single parasite. The most commonly found parasite species were Entamoeba histolytica (13.3%) and Ascaris lumbricoides (12.0%). Anemia was detected in 55.4% of the pregnant women, malnutrition in 25.0% and overweight or obesity in 24.1%. There was a statistically significant association between enteroparasitosis and schooling. However, no association of, enteroparasitosis, anemia, maternal nutritional status with the existence of a bathroom in the home was noted. Conclusions: The prevalence of enteroparasitoses and anemia is high, albeit without any association of the two conditions, while schooling was statistically associated with the presence of intestinal parasites.
