    Abstract

    Purpose

    Previous studies have shown that low birth weight (LBW) is associated with cardiovascular risk in late adulthood. Recent studies in adolescents suggest that modifiable factors may have greater influence on increased cardiovascular risk. This study aims to investigate the association between LBW and changes in anthropometric and biochemical risk factors during adolescence in a population with low average socioeconomic status.

    Methods

    In a retrospective double cohort, data of birth weight were extracted from hospital records of children born on the same day between 1992 and 2002. According to the World Health Organization, we classified the children as having LBW or normal birth weight. A total of 172 subjects among children, adolescents and adults were researched. We measured anthropometric and clinical data, lipid profile and glucose after an overnight fasting. The low and normal weight groups were compared using Mann-Whitney U, Fischer exact, Chi-square (2) and Student's t tests.

    Results

    Pregnant women with preeclampsia delivered more newborns with LBW (p< 0.001). Anthropometric and clinical parameters were similar between groups. No differences were found in the family history of cardiovascular diseases (p= 0.1), family incomes (p= 0.8) and maternal school education (p= 0.8) between groups.

    Conclusion

    In this study, LBW did not increase cardiovascular disease risk factors in adolescents. We observed absence of association between low birth weight and poor health outcomes among adolescents with low socioeconomic status from an urban city in the Brazilian northeast.

    Purpose: to evaluate the social, demographic and obstetrical profile of adolescents as compared with adult women hospitalized for abortion complications. Material and methods: this is a descriptive study that evaluated 230 women with abortion complications. Among them, 59 were adolescents hospitalized at the IMIP Maternity (Recife, Brazil) from August 1994 to July 1995. The variables studied were: educational level, marital status, any paid activity, gestation age, number of pregnancies, desire to become pregnant, use of anticonceptive method, kind of relationship, reason for voluntary interruption, clinical classification of abortion and associated complications. The procedure for data analysis was the distribution of variables among adolescents and adults, the differences being evaluated through chi² and chi² for trend. Results: compared with the adult women who aborted, the adolescents showed a lower number of paid activity and multiparity and a higher number of pregnancies resulting from an unstable relationship. Conclusions: the results indicated that biologicallly the adolescents who were hospitalized for abortion have a similar profile to adult women. What differentiates them are the unfavorable social and demographic conditions that they are faced with at their generally unplanned first pregnancies.

    PURPOSE: to analyze the association between modes of consecutive deliveries of 714 adolescents attended at the Maternity sector of the Hospital das Clínicas da Universidade Federal de Pernambuco (Federal University of Pernambuco), from January to December 2001. PATIENTS AND METHODS: according to a prospective, analytic, transversal, and incidence type study, the sequence of delivery modes was analyzed from the first to the fourth parturition of 714 pregnant adolescents, aged from 13 to 19 years (mean17.2±1.5 years). Every day, after identifying the adolescents who delivered, by the parturition room register, they were invited to participate in this study. Those who agreed, after signing a free informed consent answered a structured questionnaire with 65 direct questions with precodified closed options, among which were delivery mode and number of gestations. RESULTS: it was found that 527 (73.8%) adolescents had only one delivery, 149 (20.9%) two deliveries, 35 (4.9%) three deliveries, and 3 (0.4%) had had four deliveries. Among the 273 cesarean sections, 207 (75.8%) occurred in primiparae, 65 (23.8%) in secundiparae and one (0.4%) in a multipara. There was a statistically significant association between the first and the second deliveries for 83 (55.7%) adolescents who had transvaginal delivery, as well as for 41 (27.5%) by cesarean section. There was also coincidence about the second and third deliveries, regarding transvaginal delivery of 23 (65.7%) adolescents, as well as cesarean section of 10 (28.6%) adolescents. CONCLUSION: a tendency to coincidence of subsequent delivery modes was identified for adolescents until the third parturition.

    PURPOSE: to check whether there were differences in some social indicators between adolescent and adult pregnant women in the city of Ribeirão Preto, from January 1992 to December 1996. METHODS: the information was obtained from hospital discharge forms and was analyzed at the Hospital Data Processing Center of the FMRP-USP. The analyzed parameters were: number and types of deliveries, category of hospital admission, occupation, and obstetric diagnosis. The 6.04a text processor Epi-Info System, a data bank and statistics of epidemiology produced by the Centers of Disease Control and Prevention (Atlanta, GA, USA), and Dbase IV were used to process the information. The association between variables was tested by the chi² test, with level of significance set at 5%, using the GraphPad Prism version 2.0, 1995 software. RESULTS: a total of 43,253 deliveries occurred during this period, among which 7,134 (16.5%) corresponded to adolescent deliveries, while 36,119 (83.5%) to adult deliveries. The number of deliveries by adolescent girls increased 25.5% along this period. The proportion of adolescent deliveries in the unified health system category of admission increased, and it was higher than that of the adults'. Only 14.1% of the adolescents belonged to the economically active population, comparing with 34.8% of the adults. Only 6.8% of the adolescents were students, while 79.0% were house-workers or had a nonpaid occupation. In the analyzed period, the ratio of vaginal delivery increased among the adolescents, as compared to that of the adults. The ratio of cesarean delivery persisted stable and higher among the adults. Premature delivery and false labor were significantly more frequent among the adolescents. CONCLUSION: the number of deliveries increased among the adolescents, and most of them were normal. The ratio of admission by the unified health system category and that of vaginal delivery were higher among the adolescents. There were more adolescents without an economically active work. Thus, we recommend strategies to prevent adolescent pregnancy, mainly among the poor population.

    PURPOSE: to evaluate the nutritional condition of pregnant adolescents using the pregestational body mass index (BMI) and the BMI at the end of pregnancy and to establish a possible association with the type of delivery and weight of the newborn child. METHODS: in a descriptive retrospective observational study 558 pregnant teenagers as well as their newborns were evaluated in the Obstetrics outpatient clinic of the Universidade Federal de São Paulo, Brazil (UNIFESP-EPM), from January 1998 to December 2000. The sample consisted of pregnant girls who were between 10 and 19 years old at the time of the first prenatal examination, excluding the teenagers who had preexistent disease and those with incomplete data in their records. Thus, the sample consisted of 300 pregnant teenagers. Qualitative variables are presented as absolute and relative frequency and quantitative variables as mean, standard deviation and range. The correlation between maternal variables (pre-pregnancy BMI and final BMI) and parameters of the newborn (type of delivery and weight) was determined by the c² test and the differences were identified by partitioning of the c² values, with the level of significance set at p < 0.05 (a = 0.05). RESULTS: nutritional deviation was detected in 34.7% of the girls, at the beginning of pregnancy. Of these adolescents, 27.7% presented malnutrition, 4% were overweight and 3% were obese. By the end of the gestational period, BMI of 54.3% of them was normal, 1.3% correponded to malnutrition, 27% to overweight and 17.3% to obesity. The mother's nutritional condition (malnutrition, normal, overweight and obesity) did not affect the method of delivery, either vaginal (80.3%) or cesarean section (19.7%). The patients who reached end of pregnancy with BMI corresponding to malnutrition had 75% of neonates under 2.500 g. CONCLUSIONS: the mother's nutritional status was not related to the type of delivery. BMI corresponding to malnutrition at the end of pregnancy was significantly related to more cases of newborn babies under 2.500 g.

    PURPOSE: to evaluate the uterine volume in women between 10 and 40 years in order to observe if the uterine volume in adolescents is smaller than the uterine volume in women between 20 and 40 years. We intend to emphasize the differences between the uterine volume of adolescents and that of adult women and to correlate with the immaturity of the genital tract of adolescents regarding gestation and delivery. METHOD: a cross-sectional study, which included 828 patients between 10 and 40 years old divided into two groups and examined using abdominal ultrasound to obtain the uterine volume measure. The first group consisted of 477 (57.7%) adolescents, and the second group of 351 (42.3%) adult women between 20 and 40 years old. In the adolescent group, ultrasound examination was performed by a single observer and in the group of adult women ultrasound examination was performed by a group of observers who used the same methodology as that of group 1. Image Point HX (Hewlett Packard) and Hitachi 525 ultrasound equipment were used with a multiple frequency probe. For the calculation of the uterine volume we used the longitudinal diameter (LD), anteroposterior diameter (APD) and transverse diameter (TD) with the (LD x APD x TD) x 0.45 formula. RESULTS: adolescents aged 10 to 17 years had a smaller uterine volume than women aged 20 to 40 years (p<0.05). Adolescents who delivered twice had a uterine volume similar to that of the patients between 20 and 40 years old with respective mean values of 62.6 ± 20.6 and 69.0±22.9 (p>0.05). CONCLUSION: adolescents less than 18 years old or primiparous have a smaller uterine volume than women between 20 to 40 years old. However, adolescents aged 18 years or older, or secundipara, have a uterine volume similar to that of women aged 20 to 40 years.

    Purpose: to evaluate the incidence of side effects and acceptance (continuity rate) of depot medroxyprogesterone acetate (DMPA) as an injectable three-monthly contraceptive given to adolescents in our milien. Method: forty adolescents (70% lactating) started to use DMPA and were followed-up for a mean of 14.2 months. Spontaneous complaints, menstrual changes, physical examination and laboratory data were collected and studied using Wilcoxon or McNemar tests. Results: the most frequent complaints were abdominal pain (16.6%) and headache (15.2%). Predominant menstrual patterns were spotting and oligomenorrhea. Significant variation of the systolic blood pressure was not observed during the follow-up. There was a slight fall in the levels of diastolic blood pressure, at the limit of significance. Significant deviations from baseline regarding fasting glucose were not noted, but the mean hemoglobin concentration tended to increase. Weight gain (mean 3.9 kg at 12 months) and menstrual irregularity (occurred in more than 70% of all visits) were the main reasons for discontinuation of the method. Twenty-seven patients were accompanied during 12 months and the continuity rate at that time was 81.5%. Conclusion: depot medroxyprogesterone acetate is a satisfactory contraceptive method for adolescents.

    Purpose: the objective of the present study was to determine the number and type of deliveries, category of admission to the hospital, occupation and obstetrical diagnosis for adolescents from the municipality of Ribeirão Preto, from January 1992 to December 1996. Methods: the information obtained from hospital discharge forms was analyzed at the Hospital Data Processing Center. The 6.04a-text processor Epi-Info System, a data bank and statistics for epidemiology produced by the Centers of Disease Control and Prevention (Atlanta, GA, USA), and Dbase IV were used to process the information. The association between variables was tested by the chi² test, with the level of significance set at 5%. The analyzed parameters were: number and type of delivery, category of hospital admission, occupation and obstetric diagnosis. Results: a total of 42,969 deliveries occurred during the study period, among which 7,134 (16.6%) corresponded to adolescent deliveries. An increase in the number of deliveries by girls in this age range occurred over the years, from 1,225 in 1992 to 1,538 in 1996. Deliveries were reported starting from 12 years of age, with a gradual increase in this number, especially after 14 years of age, when a 104.2% increase occurred for deliveries at this age, followed by 48.8% at 15 years, 36.1% at 16 years, 14.0% at 17 years, 52.8% at 18 years, and with practically no increase among 19-year-old girls. The highest number of deliveries (5,709) was recorded for the unified health system category of admission, followed by 1,277 deliveries for the prepaid category and 148 deliveries for the private category. With respect to occupation, 14.1% of the patients belonged to the economically active population, while 85.8% did not. Of the total deliveries, 59.2% were normal, 5.6% were forceps deliveries, and 35.2% were cesarean sections. The most frequent obstetrical diagnoses were: problems of fetus or placenta affecting maternal management (7.9%), fetus-pelvis disproportion (6.0%), problems with the amniotic cavity and membranes (5.0%), hypertension complicating delivery and puerperium (3.5%), and premature or false labor (3.4%). Conclusions: most deliveries were normal and occurred more frequently at the end of adolescence, especially among girls belonging to the unified health system. There was a predominance of adolescents not belonging to the economically active population. Some obstetrical complications were diagnosed at the time of resolution of pregnancy.
